The Hennepin Plus Program delivers youth outreach services that combine trauma-informed, culturally responsive case coordination and mental health support to keep students in school and out of crisis. By partnering with schools in Hennepin County, we reduce the risk of homelessness, improve academic success, and increase college and career readiness—all while decreasing county involvement for students and their families.

How Hennepin Plus Works

Our case coordinators collaborate with school staff to identify students at risk of housing instability, behavioral health struggles, or academic disengagement. We create individualized plans that connect students and families with resources—such as housing prevention, food assistance, and legal aid—to address root causes of instability.




Trauma-Informed Case Coordination

Recognizing the diverse needs of Hennepin County students, we offer services in English and Somali. Our culturally affirming approach ensures every student and family feels understood, respected, and empowered to ask for help without stigma.





Culturally Appropriate, Language-Specific Support

Licensed therapists and trauma-informed counselors provide school-linked therapy, brief intervention, and mental health groups directly on campus. By meeting students where they are—between classes or after school—we eliminate barriers like transportation and reduce the shame often associated with seeking mental health care.





Onsite Mental Health Services

Through individualized coaching, we help students develop study skills, explore career paths, and prepare for college applications or vocational training. Workshops on résumé building, interview skills, and financial literacy ensure young people graduate with a clear plan for their future.




Academic & Career Readiness

By proactively addressing crises—whether related to housing, family conflict, or mental health—Hennepin Plus keeps students connected to school and community supports. This early intervention model decreases referrals to child protective services, juvenile justice, and other county systems.






Reduced County Involvement

Students experiencing trauma, housing instability, or behavioral health challenges are more likely to fall behind academically and drop out—putting them at higher risk of homelessness and decreased job opportunities. Hennepin Plus breaks this cycle by providing youth outreach and school-based mental health supports focused on stability, resilience, and long-term success.

When students have consistent, culturally responsive care and access to community resources, they’re more likely to graduate, attend college, or secure meaningful employment. By partnering with schools and leveraging local resources, Hennepin Plus creates a safety net that keeps young people moving forward.
