Our Transitional Living Program (TLP) provides youth transitional housing and wraparound support for 18–24-year-olds at imminent risk of or experiencing homelessness. With 16 scattered-site apartments, TLP serves 20 young people—half of whom are young single parents—helping them move from crisis to long-term stability. Participants choose their own affordable apartment, sign a lease, and receive a partial rent subsidy for up to three years, all while building independent living skills and financial resilience.

Our Transition Living Program (TLP) Case Managers provides ongoing support—helping youth learn to budget, cook, clean, and navigate landlord relationships. Through monthly independent living skills groups, participants hone practical skills like job readiness, credit building, and positive rental history.

Move Forward therapists offer free, trauma-informed mental health services tailored to each participant’s needs.
We connect youth to job training and employment opportunities, empowering them to increase their income, build a résumé, and secure financial independence.
Monthly group sessions create a supportive community where participants can share challenges, celebrate successes, and build lasting connections with peers and staff.

Our Transitional Living Program (TLP) breaks that cycle by providing a safe apartment, wraparound case management, and access to mental health care—all designed to create a pathway to self-sufficiency.
By combining youth transitional housing with counseling, life skills training, and financial support, we ensure participants not only find a home but also develop the tools and confidence to maintain it. At program’s end, they either remain in their current apartment or move to housing of their choice—ready to live fully independently.
